Join us for our Back to School and Sports Physicals for the 2025-2026 school year. Multiple dates and locations are available throughout summer.

What is covered?

• School physicals
• Sports physicals, if needed (Participation forms must be provided by the parent or guardian)
• Immunizations offered at select locations
  • Parent or guardian must submit shot records before appointment

Does my child qualify?

• Your child must be 18 years old or younger; be uninsured and have no other insurance options

How to schedule an appointment?

• Pop Up Locations: You can schedule your appointment for the location of your choice below.
• Shepherd’s Hope Locations: To be seen at one of the Shepherd’s Hope’s Health Centers, please call 407.876.6699

Does my child need to attend the school listed to attend a pop up location?

• No, all pop up locations and dates are open to any child that meets the qualifications of being 18 years old or younger and without health insurance.
