What is covered?
- School physicals
- Sports physicals, if needed (Participation forms must be provided by the parent or guardian)
- Immunizations offered at select locations
- Parent or guardian must submit shot records before appointment
Does my child qualify?
- Your child must be 18 years old or younger; be uninsured and have no other insurance options
How to schedule an appointment?
